Botox® is the trade name for Botulinum Toxin, a neurotoxin in the form of a purified protein. The mechanism of action for Botulinum Toxin (Botox) is really quite simple; it is injected into facial muscles but really doesn’t affect the muscle at all. Botulinum Toxin (Botox) works by blocking the transmitters between the motor nerves that innervate the muscle.
The areas typically treated with Botulinum Toxin (Botox) for wrinkles are the forehead, between the eyes (glabellar region), around the corners of the eyes (crow’s feet) and around the lips (smokers lines or gummy smile). There is no loss of sensory feeling in the muscles. Once the motor nerve endings are interrupted, the muscle cannot contract, causing wrinkles in the skin to smooth. The effects of Botulinum Toxin (Botox) last approximately three to four months, at which time the patient will require retreatment.
Botulinum Toxin (Botox) has important clinical uses as an adjunct in facial pain and bruxism cases for patients with chronic TMJ and orofacial pain. Botulinum Toxin (Botox) is also used to complement esthetic dentistry as a minimally invasive alternative to surgery.
Dysport is a Botulinum Toxin product developed in the United Kingdom during the early 1990’s. Originally created to treat neurological and ophthalmic conditions, Dysport is now known to treat a variety of conditions, including moderate to severe frown lines.
During the procedure, a small amount of Dysport is injected directly into the facial muscles that correspond with wrinkles and lines. No local anesthetic is required and patients can expect treatment to take 10-20 minutes.
The esthetic results of Dysport injections generally last 12 to 16 weeks. Fortunately, due to muscles “unlearning” the habit of frowning and/or squinting, the need for injections may become less frequent. Side effects may include; dry mouth, injection site discomfort or pain, tiredness, headache, neck pain, muscle pain, and eye or vision problems.
Xeomin (incobotulinumtoxinA), is a neurotoxin approved by the US FDA for the treatment of moderate to severe frown lines in adults. Xeomin is made of a protein purified from the bacterium Clostridium botulinum. Botulinum Toxins block nerve activity in the muscles, causing a temporary reduction in muscle activity, thus a reduction in wrinkles and lines.
Xeomin is the first Botulinum Toxin that does not need to be refrigerated before use, which simplifies distribution. It has no additives, containing only Botulinum Toxin type A – this may mean there is less risk of developing antibodies against Xeomin compared to other available neurotoxins. The effects of Xeomin are visable within one week, with results lasting 3 to 6 months.
Xeomin should not be used interchangeably with other botulinum products. There is a risk all botulinum toxin products may spread from the area injected, causing potentially life-threatening swallowing and breathing problems. Other risks may include bleeding and bruising at the injection sit
Botulinum Toxin (Botox) is used esthetically to decrease the appearance of aging by reducing muscle movement. Areas of treatment include; the glabellar area (the 11’s/frown lines between the eyes), frontalis muscles (forehead lines), crow’s feet, chin dimples, radial lip lines (smoker’s lines) and nose lines. Botulinum Toxin can also be used to reduce the muscle mass in the masseters (jaw line) for a slimming facial esthetic result.
Receiving Botulinum Toxin takes only a few minutes and there’s no need for anesthesia or down time. The injections are made using a very fine needle and are virtually painless.
Generally, results are visible within two to ten days. To reduce the possibility of bruising/swelling, it is recommended to avoid alcohol for at least 24 hours and aspirin/anti-inflammatory medications for a period of two weeks prior to the procedure. As always, consult your physician before stopping any medications.
The most common side effects of Botulinum Toxin injections are bruising, redness, and swelling. Other, rare side effects are possible and will be reviewed by the treating physician. To avoid adverse reactions/results, do not manipulate the treated area for 2 hours after injections and maintain an upright position.
The esthetic effects of Botulinum Toxin typically last three to four months. As the treated muscles regain movement, lines and wrinkles will begin to reappear and require retreatment. Over time, these lines and wrinkles should appear less noticeable due to training the treated muscles to relax.
Dermal Fillers
If you are looking for a non- surgical face lift or you just want to smooth out a few fine lines or fill in deep set wrinkles, injectables are for you. Millions of people use dermal fillers to achieve a more relaxed and smooth facial appearance while still having the freedom of expression.
As skin ages, it begins to lose collagen filled with hyaluronic acid (HA). Collagen and HA add structure and volume to the face, therefore a lack of HA causes the skin to sag and wrinkle – that’s where dermal fillers come into play.
Dermal fillers are smooth injectable gels made from naturally occurring hyaluronic acid. Dermal fillers add volume to weakened facial features and can stimulate collagen growth. Each dermal filler is unique in its own way but similar in two. First, they plump skin and gently lift it to restore collagen loss due to the normal aging process. Secondly, dermal fillers motivate the body to create its own natural collagen and restore your face to a more natural, youthful appearance.
Dermal fillers approved by the FDA are minimally invasive and very safe. As with any procedure, there are possible risks, the most common being post-procedure bruising and swelling which are usually very minimal and resolve quickly.
Dermal filler treatments typically take 20-40 minutes and the amount of dermal filler needed will depend on volume loss and the areas you would like to restore to its natural beauty. You will see results immediately after treatment.

Juvéderm is a soft but stable injectable gel made of hyaluronic acid. Hyaluronic acid is a natural substance found in the skin that assists in adding hydration and volume. The American Academy of Facial Esthetics trained professional clinicians carefully introduce Juvéderm into the mid to deep dermis to temporarily smooth wrinkles and fine lines; replenishing volume in the skin almost instantly. The dermis is part of the subsurface skin layer that includes nerve endings, blood vessels, sweat, oil glands, and connective tissue.
The Juvéderm dermal filler treatment process is very quick (about 15 minutes) with immediate results. Downtime is minimal and you can get back to your normal routine right away. The Food and Drug Administration approved Juvéderm in 2006 as the only hyaluronic acid filler that will last up to one year.
Juvéderm is a robust formula designed to correct moderate to severe facial wrinkles and folds providing versatility while restoring the contours of your face to a smooth and natural look.
The Juvéderm advantage is that its supple gel consistency provides a more natural-looking appearance than other products. Juvéderm is also long lasting so you can keep your improved appearance from six to nine months. If you have a follow-up enhancement within a few weeks after the first dermal filler treatment, the benefits could last even longer. Side effects are rare, but may include tenderness, bruising, mild pain, swelling and redness at the injection site and generally resolve within a few days.
Restylane is a non-animal, biodegradable filler gel composed of hyaluronic acid approved by the Food and Drug Administration for cosmetic injections into subdermal facial tissues. With age, skin becomes more susceptible to wrinkles and sagging. Exposure to damaging sun rays and years of muscle movement (squinting, chewing, smiling) contribute to the breakdown of tissue and loss of volume in the skin. Restylane, a soft tissue filler, adds volume to the skin via tiny injections while smoothing facial folds and wrinkles in areas around the mouth and eyes.
The average treatment is 10 to 20 minutes per session, depending on the individual and areas being treated. Restylane injections require minimal downtime, allowing patients to return to work and normal activities following treatment. Side effects are rare but include; an allergic reaction, bruising/swelling/redness at injection site, numbness, and temporary redness, swelling, pain/tenderness, or discoloration. Treatment results are temporary; typically lasting a few months to a year. To achieve optimal results, additional treatment sessions may be needed. A qualified dental or medical professional will help you determine which facial filler injections will provide optimal results.
Voluma XC is the first and only FDA-approved injectable gel filler to correct age-related volume loss in the midface. Made of hyaluronic acid (a natural occurring substance found in skin), this soft, pliable gel is indicated for deep (subcutaneous and/or supraperiosteal) injections for check augmentation.
As aging progresses, facial structures begin to sag – fat pads diminish and we are left with hollow faces, temples and jowls. Voluma XC works to reverse the signs of aging by instantly lifting, sculpting, and contouring the midface region. The injections are nearly painless as the product contains lidocaine and is injected deeper into the skin where there are fewer nerves. Side effects are moderate and generally last 2 to 4 weeks. Common side effects include temporary reactions at the treatment site such as tenderness, swelling, firmness, lumps/bumps, bruising, pain, redness, discoloration, and itching.
Voluma XC is an optimal treatment for natural-looking, youthful results that last up to 2 years.
PDO Thread Lift
The PDO Thread Lift is a treatment which lifts and tightens sagging skin tissue, using threads made of Polydioxanone (PDO). The threads introduces them into the deeper layers of the skin. Once introduced, the threads produce 3 effects in the skin:
- Instant Skin Lifting through mechanical effects
- Cellular Renewel, through Collagen stimulation, and neovascularisation to improve skin texture, fine lines and elasticity
- Skin Tightening, by contracting fat tissue
About 6 months after the procedure, the PDO threads will disappear through simple hydrolysis (they are absorbable). The lifted facial contours will remain for about 3-6 months more, due to the cellular rejuvenation effects. Your skin cells are stimulated to produce new collagen, and new blood vessels to improve skin microcirculation. At the same time, the PDO threads cause the fat tissue to contract, producing a skin tightening and facial.
PDO Threads are inserted into the skin via housing needles called cannulas, in a sterile procedure
A PDO Thread Lift is a minimally invasive procedure. After numbing and infusion of local anesthetia, the PDO threads are inserted via sharp or blunt tip needles (called cannulas) into different layers of the skin. Once inserted, the threads anchor the skin and lift it upwards. The excess threads are then cut off.
Depending on the areas treated, the procedure takes 15-30 minutes. There is some discomfort during the proceudre, but because of the improved techniques, my patients are experiencing less and less pain.
See the procedure:
There are many types of PDO threads – and this can be confusing for patients, and even doctors.
There are many types of PDO threads, for example the screw type threads for rejuvenation.
In general, PDO threads can be divided into floating, non-barbed threads and barbed threads. These can be further sub-classified in several ways:
You may have heard of fancy names like “double screw” or “tornado”, or “heart barbs”. They all refer to threads in one of the categories described above. In addition, PDO threads come in different thickness and length. These all affect their quality, tensile strength, effectiveness, and of course, price of your treatment.
Your doctor may choose to use different threads for you, depending on your skin lifting and rejuventation needs. I will not describe the different threads in detail here as it is very much a professional discussion – you can ask your doctor more during your consult with him/her.
Broadly, PDO Threads can be classified into these large groups
This treatment is surprisingly safe. Compared to fillers, the side effects are, in fact, much milder (fillers can even cause skin death and blindness). Because PDO threads are inserted via needles and not surgery, and are absorbed in 6-8 months, many of the side effects associated with older thread lifting procedures, such as infection and thread breakage are also minimised.
Depending on the types of threads used and areas treated, bruising may or may not occur. For the High Definition K-Lift which I do in my clinic, there is no to minimal bruising. Expect some mild swelling and soreness for 3 to 5 days after the procedure, and a sensation of tightness (mild) of up to 2 weeks. Temporary mild dimpling of the skin can also occur – an resolves in a few days.
Some side effects are related to the technique of insertion, and the types of threads used. These include surface depressions, thread migration and even extrustion, facial assymetry, prolonged pain, and prolonged dimpling. PDO threads are not created equal – and your doctor’s training in the procedure is also extremely important in ensuring a good outcome with minimal side effects from your procedure.
Finally, more serious side effects, though rare, can occur. These include infection, nerve damage, formation of nodules, and damage to the salivary ducts. The face has a very complex anatomical structure. Intimate anatanomical knowledge is required to perform a PDO thread lift well, and to avoid serious side effects.
Migration of unilateral, unidirectional threads in one of my patients, causing thread extrusion, 1 month after the procedure. I removed the thread for her, with no permanent side effects.
The side effects are mostly manageable – removal of threads may be required in cases such as migration and extrusion. I previously had to remove some threads from my patients because some of the PDO unidirectional barb threads did not hook on well enough. There was no scarring after removal, and the problems resolved completely. Of course, I have stopped using those threads since.
